Popes such as John Paul 2 have paid attention to it and more or less tried to do what Our Lady requested, but none the less it remains private revelation. For example, arguably Russia hasn’t been consecrated as was requested, but there is no pain of sin or any such attached to this fact.
And apparently, according to Sr Lucia (one of the visionaries) Our Lady accepted the general consecration of the world in the place of the specific consecration of Russia.
Public revelation, which ended with the death of the Apostle John (even though the apostolic beliefs and teachings are constantly being refined and developed), doesn’t chop and change in quite such a quick and arbitrary fashion.
And for anyone, including a Pope, to refuse to obey the dictates of publicly revealed (apostolic) truth (for example to deny the dogma of the Trinity or some such) would indeed incur the pain of sin.
The reason stated above is precisely why I would question living by the promises/request as seen through the vision, ie praying to Mary’s immaculate heart.A private revelation is not infallible like the Bible and that is why it does not become part of Divine Revelation. There can be errors in the transmission of a message.

Praying to the Immaculate Heart is not a requirement. If it were a commandment that was part of public revelation, then it would be absolutely neccessary.
