are you going to call them “Cuddles” or “Your Majesty” because it makes them happy and calm, or insist on treating them as an 65 year old adult named Pat regardless of how it makes them cry?
You don’t have to do either.
 
Most people I know would be kind to someone who is suffering, they would do any moral thing within their power to ease that suffering,

Heck, most people I know will feed a cookie to a stuffed dragon at a tea party if their 3 year old asks them to.
 
There are always other options and there is no obligation to call them anything. There are likely many other things you can with similar results.

Too often our perception of femininity and masculinity are set by stereotypes prevalent in one’s culture.

It’s like relying on fun hours mirrors to set your perception of yourself. Sure you know that the image as reflected by a fun house mirror is you but it’s distorted.

Same thing with femininity and masculinity. Too often what comes to the mind are the distorted versions.
 
My wife is in a situation as a teacher that she has to deal with this. What is the best approach?

I’m with your take, to an extent. We have people who have dysphoria with an unknown cause. We know that these people are at risk for suicide. Treating them with dignity and calling them by a new name seem like pretty cheap concessions to make to help them deal with their issues. The compassion might also help keep the door open to further conversations and support down the road.

I do have limits. If a young family member wants me to call ‘her’ ‘him’ I can deal with that. If she wants me to call her ‘zir’ or whatever I’m going to bow out and either use the new name or the traditional pronoun. Further, their transition doesn’t mean that they get to do things like have access to the locker room with which they identify; or compete in the gender sports with which they identify. I don’t say this to be cruel, but rather to be as fair as I can to everyone. It also is respectful of the biological sex with which they were born.
 
Well, I’m sure we’ve run into men who act like they are PMSing and need to pop a Midol or two (I know I have).

Considering that they are now engaged in winning at various sports previously designated as “women’s”, the answer would appear to be “Most likely”.

That does not appear to be the way courts may be moving. Although I don’t make a point of watching cases, I came across one recently of a funeral home director who faces with a biological male who insisted they were a woman and had the right to dress as one; employment/discrimination law appears to be headed that way.

The funeral director’s position was that the matter was upsetting to those using his services; they were grieving and being confronted with an employee who was clearly not the female they were trying to appear as.

No one is all that sure where this is going to go; as an employee you may be required to treat someone with transgender dysphoria in ways you have not contemplated, or be faces with a discrimination lawsuit - even as an employee; or you may cause your employer to face one.
 
The ACLU is rabidly anti-Christian (especially anti-Catholic), and pursues a rabidly far left political agenda. They SJW’s, not human rights activists.
 
And the best way to help our brothers and sisters who feel alienated is to make them feel accepted, loved, respected.
Unfortunately, what they require to feel “accepted, loved, and respected” requires that we show the exact opposite to some other people.

So a young man who says he is a woman wants to change in the girls locker room… to show him acceptance, love, and respect, we have to let him use the girls locker room, but that means we are being unaccepting, unloving, and unrespecting of the real girls in the locker room.

And that goes at least quadruple for women in domestic violence shelters.

In my previous work place, the first aid cabinet was stocked with Midol.

We had problems with constantly running out of this stuff which was puzzling since there were about 30 people at most, 3 women and the rest men.

It turns out that it was men who liked taking Midol since it was effective in curing headaches. We worked at very high altitude so headaches were a common problem.
 
People who have something going on at the genetic level that science has not yet fully uncovered

They are people who have a mental illness
There is a third possibility.

Something going on at the hormonal level.

Let’s face it. Hormones have a huge effect on the brain and hormones have an even greater effect on the developing brain before birth.

It seems like hormone levels in the intrauterine environment have a huge effect on the developing embryo.
 
Good point. The issue of trans-genderism does invite the question, “What does it mean to be a man, to live as a man? What does it mean to be a woman, to live as a woman?” Is being a man no more than wearing pants and watching sports? Is being a woman no more than wearing dresses and lipstick? So much of it seems based on shallow and superficial stereotypes.
